Pre-Registration for
Newborn Families
Having a Newborn soon? Save your spot in the practice as we limit how many newborns we take per month to ensure the best possible quality of care.

*$100 registration fee to hold your spot

To register:
1. Click the enrollment link
2. Add a parent as the first caregiver but click "I am enrolling others but not myself".
3. Then add a member - your infant. Write Infant + Your Last Name, put in todays date as the birthdate (we will change this later), choose a gender (can be changed later)
4. For Membership Start Date, choose your due date.
General Registration
Ready to Join? We are excited to have you! It takes about 5 minutes to enroll and save your spot in the practice.

*Currently the family maximum of $250 is not working on the site. If your family is over $250 please pick a future start date of 1-2 days from now and we will manually change the charge.
College Students
Have a College Student and want access to a Physician by telehealth or in person when they are home or nearby? Not ready to switch to an adult doctor? This plan may be right for you!
